Health assessments provide time to review your health, discuss concerns and plan any care you may need. At Springwood Mall Medical Centre, our nurse and doctor work together to assess your individual needs.
Health assessments available
- Health checks for 3-year-olds: discuss your child’s health, development and any concerns.
- Ages 45–49: health assessments for people identified by their doctor as being at risk of developing a chronic disease.
- Ages 75 and over: review your health, medicines, day-to-day functioning and support needs.
- Aboriginal and Torres Strait Islander health assessments: personalised assessments focused on your health priorities, wellbeing and ongoing care.
Our team can advise which assessment is appropriate for you.
What to expect
Your appointment usually starts with our nurse. Please allow 45 minutes to one hour with the nurse, followed by time with a doctor.
The doctor will review the assessment with you and recommend any appropriate next steps, such as health advice, further checks, referrals or follow-up care.
Book by phone
Call 07 2802 2064 and let reception know which assessment you need, so the nurse appointment and doctor review can be arranged.
